Membership is $11 and there’s no paper form: you join online in about two minutes.
Auction item donation form
Donating something to the Grown-Ups’ Night Out auction? Fill this out and send it to the fundraising team.
For volunteers & committee chairs
Reimbursement & payment request
Fill out the whole form before printing, then sign it and send it to the PTA treasurer with original receipts. All purchases must be budgeted and approved in advance.
SMMUSD facility use permit
For PTA event chairs: apply through the district, and confirm details with the PTA president and treasurer before you submit.
PTA officer nomination
Elections are in March. A nominating committee forms in November or early January and presents a slate at least 28 days before the election.
Honorary Service Award nomination
Each spring we recognize people whose service to students goes above and beyond. Nominations open in the spring, and awards are presented in May.
